These terms govern your use of the Form Slots add-on and website ("Form Slots", "the service", "we", "us"). Form Slots is operated by Leandro Zubrezki, a sole proprietor based in Argentina. By installing or using the service, you agree to these terms. If you do not agree, do not use the service.

1. Using our services

You may use Form Slots only as permitted by these terms and by applicable law. Do not misuse the service, interfere with it, or try to access it using a method other than the interface and instructions we provide. You may use the service only in connection with forms you own or are authorized to manage. Do not use our name or branding without permission, and do not remove or obscure any legal notices.

2. Privacy

Our Privacy Policy explains what data we collect and how we use it. By using Form Slots you agree that we can collect and use that data as described there.

3. The add-on and software

We grant you a personal, worldwide, royalty-free, non-exclusive license to use Form Slots as provided. This license is only to let you use the service as these terms allow. You may not copy, modify, distribute, sell, lease, reverse-engineer, or attempt to extract the source code of the service. We may add, change, or remove features, and we may update the software from time to time.

4. Payments and plans

Form Slots offers a free plan and paid plans, described on our pricing page. Paid plans are billed monthly or yearly and renew automatically until cancelled. You can cancel at any time; your paid features remain active until the end of the period you have already paid for.

Prices are shown in US dollars. Any applicable sales tax or VAT is calculated and collected by Paddle at checkout.

5. Refunds

If for any reason you are not satisfied with the service, contact us at support@formslots.com and we will work with you to resolve it. We handle refund requests on a case-by-case basis. Refunds and billing support are also handled by Paddle as our Merchant of Record.

6. Warranties and disclaimers

The service is provided "as is" and "as available", without warranties of any kind, whether express or implied, including any implied warranties of merchantability, fitness for a particular purpose, or non-infringement, to the fullest extent permitted by law. We do not warrant that the service will be uninterrupted, error-free, or that it will meet every requirement.

7. Limitation of liability

To the extent permitted by law, we are not liable for any lost profits, revenues, or data, or for any indirect, special, consequential, or punitive damages arising out of or related to your use of the service. Our total liability for any claim relating to the service is limited to the amount you paid us for the service in the twelve months before the claim.

8. Business use and indemnification

If you use Form Slots on behalf of a business, that business accepts these terms and will indemnify and hold harmless Form Slots and its operator from any claim, proceeding, loss, or expense arising from your use of the service or your violation of these terms.

9. Your data

Your form responses remain in your own Google account at all times. Backup Sheets created during a reset are owned by you and stay in your Google Drive. After you stop using the service, we retain the data we hold for up to 30 days and then delete it, as described in the Privacy Policy. You are responsible for keeping your own backups.

10. Changes to these terms

We may modify these terms from time to time. For material changes we will give reasonable notice, for example by posting the updated terms here and changing the date above. Changes required for legal reasons may take effect immediately. If you continue to use the service after changes take effect, you accept the updated terms.

11. Governing law and jurisdiction

This agreement is governed by and interpreted according to the laws of Argentina, and any dispute arising under it, including non-contractual disputes or claims, is subject to the exclusive jurisdiction of the Argentine courts.
